About You In Home Care is seeking a dedicated Licensed Practical Nurse (LPN) to oversee and support our team of CNAs and caregivers in Jackson, MS. This role is crucial in maintaining high-quality patient care, ensuring proper documentation, and upholding compliance with regulations and care plans. The LPN Supervisor will report directly to the Clinical Director and play a leadership role in daily operations.

Responsibilities:

• Supervise and support caregivers – Provide training, mentorship, and oversight to ensure quality care and adherence to policies.
• Monitor patient health – Conduct regular assessments, document changes in health status, and update care plans as needed.
• Ensure compliance and accuracy in documentation – Maintain thorough records of patient care, interventions, and observations in accordance with state and company standards.
• Administer medications and treatments – Ensure safe administration of medications and treatments as prescribed.
• Act as a liaison – Communicate effectively with patients, families, caregivers, and healthcare providers to ensure continuity of care.
• Promote a positive work environment – Foster teamwork, encourage professional development, and address any concerns among staff.

Qualifications:

• Current LPN license in Mississippi
• Experience in home health, long-term care, or a supervisory role preferred
• Strong documentation, organizational, and leadership skills
• Ability to manage multiple responsibilities and make sound clinical decisions
• BLS certification required
